Healthcare providers can send requisitions directly to Merivale Medical Imaging.
Once the requisition is received, it can be used to process the requested imaging service.
2. What You Need to Know
MMI accepts requisitions through several methods, including physician referrals and electronic referrals.
Receiving a requisition does not mean you should assume an appointment date or time unless that appointment has been confirmed.
3. After Your Requisition Is Sent

If your healthcare provider sent it successfully, it may already be on file.
