Do I need a requisition for diagnostic imaging? Who can provide my requisition?
1. Overview
A valid requisition is required for most diagnostic imaging examinations at Merivale Medical Imaging.
The requisition tells us which examination has been requested and provides the information needed to process your imaging request.
2. What You Need to Know
For OHIP-covered diagnostic imaging, your requisition must come from an authorized healthcare provider.
Certain screening programs, including eligible Ontario Breast Screening Program appointments, may have different referral requirements.
3. Requisition Requirements
Do I need a requisition?
Yes. A valid requisition is required for most diagnostic imaging examinations, including MRI, CT, X-ray, ultrasound, mammography, BMD, CT, MRI and Nuclear Medicine.
Who provides the requisition?
Your referring healthcare provider completes the requisition and indicates the examination being requested.
4. Before You Contact MMI
Check whether:
- You already have a copy of your requisition
- Your healthcare provider has sent the requisition to us
- The requisition is complete
