Can I Use Another Clinic’s Requisition?
Does my requisition need to be on an MMI form? Can I use a requisition that names another imaging clinic?
1. Overview
Your requisition does not need to be written on a Merivale Medical Imaging requisition form.
We accept valid requisitions from licensed healthcare providers when the required patient, clinical and referring-provider information is included.
2. What You Need to Know
You do not normally need to return to your healthcare provider simply because the requisition was printed on another clinic or hospital’s form.
The requisition itself must still contain the information required for the requested examination.
3. Using an Outside Requisition
Does it need to be an MMI requisition?
No. We accept requisitions that have been completed on other valid requisition forms.
What if another clinic is named on it?
You may still submit the requisition to us for review.
If there is any question about whether the requisition can be accepted, our team can review it before booking.
